Super common. When I air sealed and insulated my attic a few years ago
there were 3 or 4 uncovered junction boxes. And due to the obvious age of the boxes and the wires I could tell they were from the original construction (house was built in mid-60's).
Of course that's still better than what happened afterwards. At some point the previous owner added a dozen recessed lights to the living room and kitchen. And they didn't even believe in junction boxes. I had wires attached with just nuts sitting loose on top of the insulation. They didn't even wrap them in electrical tape or anything. So the insulation project took 10 times longer than it should have while I dug through all the old fiber fill insulation and corrected all the electrical problems.
