1) runners on 1st and 2nd (or bases loaded, which by definition has runners on 1st & 2nd), 2) less than 2 outs, and 3) a fair fly ball that in the umpire’s judgment can be caught by an infielder making an ordinary play The rule is there to prevent an infielder from deliberately not making an easy catch in order to get a double play.
