Playing hurt could have introduced some bad habits that he hasn't resolved

If it was hurting to throw or run, he may have changed his mechanics to alleviate the pain. It may have been intentional or likely was without even realizing it. But it becomes a habit that requires first identifying it and then re-training to change it.
